• The only personal information collected is what is necessary for you to purchase the products and services you apply for.

• That information will be shared with the product provider (i.e. the specific insurance or invest-ment company) and our intermediaries ( Manulife Securities, PPI Financial, HUB Financial or Financial Management Brokerage).

• The mode of delivery of confidential information shall be authorized by you. Your verbal or written permission will be required to use fax, email or the postal service.

• In the case of an insurance application, your written permission is required for the insurance company to get information from paramedical companies, hospitals, doctors, the Medical Information Bureau (MIB), other insurance companies, financial institutions, motor vehicle reports and employment information.  This is included in the application.

• The insurance company will either deal directly with the entity involved or make their requests via a company specializing in the transfer of confidential information (Keyfacts or Watermark). At no time does Dupuis Langen or any of its employees have access to that information.

• No information collected will be used for any unauthorized purpose.

• Information may be kept on file to expedite the completion of forms in the future.

• Personal information on file locked up after office hours.   The appropriate security measures (such as firewalls, passwords, etc.) have been take with regard to electronic records.

•  Information that is no longer needed is shredded or stored in a secure location.

• You have the right to access the personal information that the insurance company, financial institution or Dupuis Langen has about you in their file. If any of the information is incorrect, you have the right to have it corrected.  You have the right to have information kept on file destroyed.

• Please report any actions you feel violates your right to privacy to Dupuis Langen's privacy commissioner.
